HOW TO CHOOSE A BOARD-CERTIFIED PLASTIC SURGEON

February 8, 2020

The world of advertising and social media has brought physicians' names to the public continuingly. Years ago, if a doctor called him/herself a cosmetic surgeon, the patient knew precisely what that term meant.  Doctors in various sub-specialties such as dermatologists, gynecologists, ophthalmologists, and even dentists are performing cosmetic surgery procedures.  This can leave prospective patients utterly confused about how to find the best and most qualified surgeons. But how, wonders Dr. Manish Shah who is an Ivy League educated Board-certified Plastic Surgeon in Denver, "is a prospective patient to know who is a diligent, excellent, caring physician and who is a charlatan?" Dr. Shah wants to disseminate information the public can use and heed as caution when selecting a cosmetic surgeon in a country now brimming with doctors who call themselves "cosmetic surgeons."

"Public law and policy do not necessarily protect us from unreasonable and untrue claims," says Dr. Shah.  "A physician needs to be licensed in medicine -- which means earning a recognized medical degree, From that point, he or she can hang out a shingle in any specialty he/she chooses; the state does not control what area of medicine he/she practices or which type of specialist they claim unless there are complaints and they are brought before the Office of Professional Medical Conduct. Often, by then, it is too late."

What can we look for before placing ourselves under a physician's care to ascertain the qualities and qualifications of that physician?  There are four basic areas that must be investigated to determine the qualifications of physicians. These are: a physician's training; a physician's certification; a physician's associations; and a physician's attitude toward his or her patients and the medical community at large.

"A physician should have and be willing to provide evidence that he or she has trained in the area of their expertise," says Dr. Shah.  "For example, patients want to locate a plastic surgeon who trained in plastic surgery, which means a post-surgical residency fellowship in the sub-specialty.  Each residency or fellowship program gives a certificate certifying the dates of service as well as the hospital and department in which the physician trained."  Physicians are usually quite proud of their achievements and keep these and many other framed diplomas in their offices.  If they are not available, then at least a curriculum vitae or a brochure describing a doctor's training should be available and should be carefully examined by the patient.  "If a physician is either unable or unwilling to disclose this information, it becomes a clear issue of 'medical consumer beware,' says Dr. Shah.

After training in their chosen field, each medical or surgical sub-specialty has a certifying board that is approved by the American Board of Medical Specialists (ABMS).  After spending the requisite number of years training, a physician has to take an examination in order to be certified.  After being certified, a surgeon often applies to the American College of Surgeons for a fellowship.  These credentials can be checked in libraries, hospital libraries, and online.

"For example, The American Board of Cosmetic Surgery is not approved by the American Board of Medical Specialists and neither are a host of other so-called 'boards,' advises Dr. Shah, who is board-certified with the American Board of Plastic Surgery which is the only plastic surgery board recognized by the ABMS.  A gynecologist, ENT or general surgeon may be an exquisitely trained physician--but if they did cosmetic surgery, where did they learn to do it?  Was it on-the-job training, or an online or weekend course? How often do they go for a refresher course?  Are they affiliated with a certified hospital in case of an emergency? "

Dr. Shah advises the would-be medical consumer "not to be impressed by a P.C. after the physician's name--it only means that they are incorporated. Jane Doe, M.D., F.A.C.S.', on the other hand, actually tells one something about the qualifications of the physician." It means, Fellow of the American College of Surgeons) It indicates to the patient that the surgeon has passed a thorough evaluation of both professional competence and ethical fitness.

If patients are still confused, Dr. Shah advises them, "check a third area: A physician's associations and affiliations.  If a physician is on the staff at a well-respected local teaching hospital in the department that is appropriate to their qualifications, then patients at least know that his or her qualifications have been appropriately researched by the hospital's committees and should, therefore, be valid and current.  "Today, however, because of skyrocketing hospital costs, the trend is toward more out-patient surgery and care," says Dr. Shah.  "Because of this, physicians have moved further away from the hospital.  This may be good for the patient from a cost standpoint, but a well-qualified physician--especially a surgeon--should maintain a relationship with a hospital just in case there is a problem with a patient that requires hospitalization."   Dr. Shah further advises patients to remember that, "when a physician is outside of an institutional setting, there is no quality control or peer review other than what the physician sets for himself."

Patients must look into many areas of a physician's training, especially certification, association, and personality prior to allowing a medical professional to manage their health care.  "If you do your homework well," says Dr. Shah, "you should end up with a compassionate well-trained competent physician with whom you feel totally at ease and in whom you have the utmost confidence."

Check if your plastic surgeon is board-certified

Since 1937, the ABPS has issued 9,499 certificates to plastic surgeons who met high standards in training requirements and successfully passed the examinations covering the breadth of Plastic Surgery. Approximately 6,900 are actively practicing.

PLASTIC SURGEON WARNS: "SUNBURN TATTOOS" COULD BE THE WORST VIRAL CHALLENGE EVER

July 28, 2019

Instagram is full of all kinds of viral challenges. Some of them are charitable such as “The Ice Bucket Challenge,” some are silly like the Kylie Jenner 'Lip Challenge,' others are fitness-oriented such as planking in obscure places. The sunburn tattoo challenge which has its own hashtag on Instagram is downright dangerous. Dr. Manish Shah is a board-certified Denver, Colorado plastic surgeon. As a father of three teenagers, he is all too aware of this alarming summer trend.

What it is: Young people are putting stencils or stickers on their skin, then they lay in the sun without sunscreen. After they burn and take the stencil off – what’s behind is an outline known as the “sunburn tattoo.”  Other sunbathers do the inverse and use a stencil to hide some skin from the sun while burning the surrounding area.

The dangers: Dr. Shah says, “At first glance, it may look like harmless fun in the sun. The bottom line is that sunburn tattoos are extremely dangerous and can put you at serious risk of skin cancer.” What makes this more dangerous than a ‘standard burn’ explains Dr. Shah is that for the stencil, or tattoo, to be visible, you would have to damage the surrounding tissue enough that it either tans heavily or burns. By intentionally keeping the area sunscreen-free will do harm to the DNA in the skin cells.  Dr. Shah warns that “sitting out in the sun without the appropriate protection could not only cause life-long damage, but could also double the risk of skin cancer.”

Is there a safe way to get a sunburn tattoo? Dr. Shah has three kids, and he would rather see them get an actual tattoo than use this method. “There is no safe way to get a sunburn tattoo. That is a major misconception. There is no such thing as a safe tan. Kids are doing a lot of these sunburn tattoos. When you have young skin like that, it is especially vulnerable to ultraviolet rays. You are more likely to see signs of aging.”

A Safe Way: You can order all kinds of temporary designs online, ask for an airbrush design from a tattoo shop, and if you want the tan tattoo look, just put a sticker on your arm before rubbing on self-tanner.

THE HOTTEST TRENDS IN SKINCARE FOR 2019. SHOULD YOU USE THEM OR LOSE THEM?

April 21, 2019

Each year brings with it fashion trends, decorating trends, and beauty trends.

The latter covers everything from nails, haircare, makeup and skincare. With the ubiquity of social media, beauty users now can pick up popular trends from anywhere across the world and follow them. Skincare is the biggest segment in the beauty industry with global sales that hover around $130 billion in 2019. There is so much for consumers to digest and decide on. We turned to Dr. Manish Shah, a Denver Board Certified Plastic Surgeon and skin care expert. He takes a microscope to some of the hottest trends in 2019 to give you insight into what’s worth your hard- earned dollars.

CBD In Skincare

Much like it did in 2018, CBD is taking off in the new year. In fact, CBD (short for cannabidiol) has no plans of easing up its reign on the skin-care world in 2019. As a case in point, there are at least two MAJOR skin care brands that now include CBD in their skincare, not to mention the indie companies who have created CBD skincare. Dr. Shah’s take: “As far as it being good for skin, it acts as both an anti-inflammatory agent and an oil reduction agent. Theoretically, its addition to skin care products, especially those tasked to fight acne and other inflammatory skin conditions, is probably legitimate. It is also a good antioxidant that can help protect against free radical damage to skin cells. CBD can be made from hemp which helps skincare companies get around the federal ban on marijuana. Side effects of unregulated use include: Nausea, fatigue and irritability. CBD can increase the level in your blood of the blood thinner coumadin, and it can raise levels of certain other medications in your blood by the exact same mechanism that grapefruit juice does.”

Microneedle Patches

Some beauty editors and bloggers swear by pimple patches. According to experts, those patches are about to get even more advanced by way of a ton of tiny microneedles (or "microstructures").The logic is that you can out put a smaller amount of acne-fighting ingredients into these tiny little cones and apply it to the skin, it's a better, more effective delivery system.

Here's how they are purported to work: The small band-aid-like sticker has spikes coated with hyaluronic acid that are thinner than a hair follicle. Through these teeny painless pricks in the skin, the patch drives the active ingredients deep in to the skin. Dr. Shah’s take:

“There is plenty of good medical literature on the validity of these small patches that are impregnated with various chemicals. The patch gets worn and the needles (either metallic or made out of absorbable polymer) penetrate the skin delivering the chemical treatment. Their design takes advantage of the concept of transcutaneous delivery of drugs. Patches can be custom made to fit various areas of the face and deliver anti-aging or other chemicals while a patient sleeps. The efficiency of delivery is better through the tiny skin punctures than if you only put the chemical directly on the intact skin. In theory, aesthetic practitioners can make custom topical treatments and place them on the patches to deliver customized at home skin treatments for their patients.”

Is a Cryotherapy Facial Is the Best Way to Brighter, Tighter Skin?

Cryotherapy has gained popularity in recent years with spa treatments exposing clients to subzero temperatures to help relieve pain and improve their health. This wellness treatment has recently undergone another adaptation with the cryotherapy facial treatment. The Cryo Facial is a cryogenic treatment that is performed by what is considered a "cryo probe," which beams vaporized liquid nitrogen across the forehead, cheeks, nose and chin. Different from the cryotherapy chambers that can be used for pain relief, the facial targets helping the face look younger. Dr. Shah’s take: “This may be more suspect. There isn't a great deal of study-based evidence that cryotherapy facials actually do what they claim. Typically, cryotherapy uses extremely cold liquid nitrogen to freeze exposed skin cells to kill them, like a wart. The facials use the same liquid nitrogen as a spray, but the aesthetician doesn't stay in one area too long to avoid frostbite to the skin. Some level of cold injury occurs, probably to a very superficial level of the skin, so there may be some exfoliation. But there are safer ways to get exfoliation without risking frostbite or hyperpigmentation.”

ANTI-POLLUTION SKIN CARE

Your skin is exposed to environmental aggressors on a regular basis. Although unseen, these pollutants can wreak havoc on your skin by breaking down collagen and elastin, the fibers that give skin its bounce. To help reduce these unwanted side effects, anti-pollution skin care products are continuing to gain favorability among consumers. Just as SPF is now de rigor in skin care, this seems to be the case with anti-pollution ingredient. Dr. Shah’s take: “I think that this is a new name for old tech. In brief, our skin is exposed to environmental contaminants that create inflammatory conditions. These conditions lead to build up of free radicals in the skin. The ingredients in antipollution skin care are basically strong antioxidants that protect against the free radicals. But the skin is assaulted by more than chemicals. It experiences dehydration, UV exposure, temperature changes, etc. True antipollution skincare should guard against all this. Typical protective and reparative ingredients include vitamin E, vitamin C, retinoids, hyaluronic acid, zinc oxide, vitamin B3, and bisabolol.”

PLASTIC SURGEON GETS REAL ON THE SECRETS MOST KEEP UNDER WRAPS

March 30, 2019

With popular TV shows such as “Botched” and the old favorite NipTuck, plastic surgeons are seen as the perfect specimen of looks, wealth and a fabulously crafted and curated lifestyle. They’re impeccably dressed, have gregarious personalities and their significant others are the epitome of perfection. Is there more to the story? Are there practice secrets that are kept more confidential than the facelift of a 50-year-old socialite? Dr. Stanley Poulos, a San Francisco Bay area board certified plastic surgeon gets real on secrets many   plastic surgeons prefer to keep under wraps.

1)Sometimes you will have more than just “a little discomfort” managed with some light dosage of pain medication.

Everyone has a different pain threshold and post op bruising and swelling vary so certain procedures will take longer to recover from. Abdominoplasty, body lifts, and combination surgeries like “mommy makeover” are just a few examples of procedures where recovery may be extended.

Many people who have had procedures can think of a time a few days post op where they wanted to curse their surgeon or ask “why in the world did I do this?”

Just because it’s an elective aesthetic surgery doesn’t make it any less of an operation. Dr. Poulos explains that there are two very important things you want to get clear on, the actual level of pain and recovery time. Recovery time to return to ranching or running marathons may be a lot different than being able to drive or get out.  He advises speaking to a few friends or references who had the same procedure. Just remember that like childbirth as time goes by the experience may seem easier than it was.

“You need to know so you can make necessary arrangements with work and childcare or elder care if need be. Some procedures can take at least a week or two before resuming normal activity so the more realistic the expectations the better,” adds Poulos.

2) There may be more than just “minimal scarring.”

“Scarring has nothing to do with the surgeon’s skill and everything to do with the patient’s genetics,” clarifies Dr. Poulos. He goes on to explain that a highly skilled plastic surgeon can do the same exact procedure with closure on two different people. One will have minimal scarring while the other will not.” People with darker complexions should consider this before deciding to do a procedure and certainly consult with your doctor’s team who can recommend remedies that help with scarring.

3) Cellulite removal isn’t just easily “zapped away” with a laser.

Today there are many options available, each claiming to treat cellulite effectively. Some are a bit more invasive. Cellulaze for example is a highly promoted actual surgical procedure using an invasive laser that must be threaded under the skin. “Even though this is considered minimally invasive, it’s not an easy zap. Even with improvement the results often will not meet expectations so be realistic.

4) There’s an expiration date on results due to the aging process.

We see celebrities who desperately battle mother nature. Aging is inevitable. The average "shelf life" of a facelift is about 10 years. “A facelift is a large financial investment along with significant recovery time. The better the skin elasticity the closer to a decade you’ll get. Odds are even better you’ll have a more enduring result if you don’t smoke or sun-worship,” says Dr. Poulos. He goes on to say that he believes it’s really important to lay out the real expectations given skin texture and volume. Some people may notice the lower quadrant of their faces aging just a few years post facelift. While a person will still look youthful post facelift, it’s not going to prevent aging. As they say “ageing is not for sissies but it sure beats the alternative.”

TALKING SKINCARE WITH DR. PAUL NASSIF

June 7, 2018

We have been a fan of Dr. Paul Nassif since we first started seeing him on BRAVO's Real Housewives of Beverly Hills where he introduced us to his world of Plastic Surgery as well as being a skin care expert. Whether we've seen him on a number of talk shows or on his addictive series on E! Botched with Dr. Terry Dubrow, that focuses on clients who are looking to have their visions become a reality after attempting surgeries that were not done appropriately.

Dr. Nassif created a skin care line that we wanted to know more about and are ecited to find out more about how this can go within your beauty routine to ensure that you keep that dewy glow that we're all looking to maintain.

ATHLEISURE MAG: For our readers that may not be familiar with you, can you tell us a little bit about who you are, and how you became a Celebrity Plastic Surgeon and Skin-care Expert?

DR. PAUL NASSIF: I’ve been practicing as a world-renowned facial plastic surgeon for over 20 years, and I’ve been working on my skincare line, Bio-Clock Dermaceuticals, for over 5 years. I juggle surgery 5 days a week, filming the E! TV show, Botched, and traveling to appearances on HSN and TVSN (Australia) frequently throughout the year.

AM: When did you realize that you wanted to create a skincare line?

DR. PN: After years of performing facial plastic surgery, I knew that I wanted to create a line of products that could help not only reverse the signs of aging, but most importantly, prevent them.

AM: What is in the current collection of your line and what does it do for the skin?

DR. PN: Bio-Clock Dermaceuticals was born out of a desire to change the skin from the inside out. These products are full of actives that address common issues, such as dehydrated skin, and reverse signs of aging.

AM: It seems that these products take care of the issues that many are concerned about from lines, under eyes as well as the neck - are there other skin concerns that you will tackle with future products?

DR. PN: We are working on adding a product with SPF to our line. I often see patients with major sun damage, which is hard to reverse. It’s very important to take the steps necessary to prevent sun damage.

AM: As people go from their 20's, to 30's, 40's and 50s what are preventative measures that users of your products can do to ensure that they are maintaining a beauty routine that makes their skin look supple and radiate a glow?

DR. PN: I always tell people to ensure that their routine contains an exfoliant, a super-hydrating serum, and an under eye cream that reduces dark circles and fine lines.

AM: How important is hydration to the body and does the skin take in moisture differently as it ages?

DR. PN: As you age, your skin requires more hydration to counteract the natural occurrence of fine lines and wrinkles. Using products that contain Hyaluronic Acid, such as my Hydro-Screen Serum, is the key to hydrating your skin and reducing those pesky wrinkles, ensuring that your skin is smooth for years to come.
